Preparing For Your Wellness Exam
How to Prepare for Your Wellness Exam:
1) You may choose to download and print the wellness exam health history form below and either mail or fax it to our office. You may also bring it with you on the day of the exam. If you do not complete the form in advance, please plan to come to our office about 15 minutes prior to your appointment to complete your health history.
2) Please fast beginning at midnight the night before your appointment from everything except any regular medications, water, and black coffee. Our office will provide you with complementary snacks and beverages after blood-work.
3) You are welcome to have fasting blood-work drawn in advance of your exam. If you desire to come in at 8 a.m. a week in advance, your physician will have all of your results the day of your exam. You may also come in at 8 a.m. on the day of your exam even if your exam is scheduled for a later time. This allows you the opportunity to eat more regular meals on your exam day.

Appointment for annual wellness exam:
Typically your wellness exam will be scheduled 2-4 weeks from the day you call, however many people prefer to schedule their annual wellness exam a year in advance.
New patients:
